Specified for the load. A driveway built for a work truck costs more than one built for two sedans, and it should, because it is a different structure.
Site preparation and grading
How much correction the site needs before anything can be poured. A flat, well draining, accessible site is the cheap case. A site that needs regrading, drainage correction, or significant fill is not.
Demolition and haul-off
On replacement work, breaking out and removing the existing slab is real cost, and it varies with thickness, reinforcement in the old slab, and how far the debris has to travel from the work area to the truck.

How do I compare two concrete quotes fairly?
Put them side by side on base material and depth, compaction method, slab thickness, reinforcement type and spacing, joint layout, drainage work and who pulls the permit. If one bid is materially cheaper and cannot answer those, you have found the difference.
Does a bigger patio cost proportionally more?
Usually less than proportionally. The fixed costs of getting a crew, forms and concrete to your property are spread across more square footage, so the rate per square foot tends to improve as the area grows.
