Why does concrete crack in Indiana?
Concrete in central Indiana cracks mainly from freeze-thaw cycling and movement in the clay-heavy soil beneath the slab. Water gets into or under the concrete, freezes, expands and has to push something. Repeated cycles do the damage, not any single freeze. A properly compacted base, correct drainage and control joints cut on a plan are what prevent it.
The two causes behind most failures
Freeze-thaw cycling and uneven soil beneath the slab account for the large majority of cracking and heaving in this climate. Water gets into or under the concrete, freezes, expands, and has to push something. If it is under the slab, it lifts. If it is in the surface, it spalls. Why clay soil makes it worse
Much of the soil across the Indianapolis metro is clay heavy. Clay holds water rather than draining it, and it expands and contracts seasonally as its moisture content changes. A slab sitting on clay that is doing that is a slab being pushed unevenly from below, year after year. This is why the same driveway detail can perform differently on two lots a mile apart.
Base preparation is the actual variable
Here is the uncomfortable part: the difference between concrete that lasts and concrete that does not is mostly decided before a single yard of concrete arrives. A properly compacted aggregate base at the correct depth drains water away from the underside of the slab and distributes load evenly. A base that was rushed, under-compacted, or shorted on depth does neither. It is invisible in the finished product and it is the first thing cut when a bid is being squeezed.
Control joints do not prevent cracking
This is worth stating plainly because it is widely misunderstood. Concrete is going to move. Control joints do not stop that movement, they decide where it shows up. A joint cut to the correct depth on a deliberate plan gives the slab a weak line to crack along, below the surface, where you will not notice it. No joints, or joints cut too shallow or in the wrong places, and the concrete picks its own line across the middle of the slab.

Drainage is the cheapest insurance you can buy
Where the roofline dumps, which way the yard falls, and whether the slab pitches water toward the house or away from it are design decisions, and they cost almost nothing to get right at the start. Is it normal for new concrete to crack in the first year?

Can cracked concrete be repaired or does it need replacing?

Does rebar stop concrete from cracking?
No. Reinforcement holds a slab together across the cracks concrete naturally develops, keeping the pieces working as one structure. Expecting it to eliminate cracking is expecting the wrong thing from it.
